TNVBC 2026-2027 Participation Release Form

By use of the facilities provided by Tampa North Volleyball Club, Inc. the undersigned expressly agrees neither the Building Owners, Club nor any member, manager officer, contract laborer, employee, agent or affiliated person or entity (hereinafter collectively referenced as “TNVBC”) shall be liable for any damages, including theft of personal property, arising from any and all personal injuries or death sustained by the “Player” (Named below) or the Player’s parents, siblings, family members or guests (hereinafter collectively referenced as “Releasor”) in connection with, arising from or related in any manner with the Player’s participation in, travel to or from or attendance at TNVBC practices, tournaments or events, including, without limitation, any damages, personal injuries or death occurring on or about TNVBC premises or facilities at any time, together with all demands, rights of action or causes of action, present or future, whether known or unknown, anticipated or unanticipated (hereinafter referenced herein as “Claims”). By their signature, the Releasors, on their own behalf and on behalf of all other Releasors, further agree that TNVBC shall not be liable for any Claims, and Releasors hereby jointly and severally agree to indemnify and hold TNVBC harmless from and against any Claims, including attorneys’ fees and costs arising out of or related to such Claims. Releasors jointly and severally assume full responsibility for any and all injuries, damages and losses which may arise from or relate to any Claims whatsoever and hereby fully and forever release and discharge TNVBC from any and all Claims, regardless of any negligence of TNVBC.

The undersigned warrant, represent and agree that they are the sole parent(s) or legal guardian(s) of the Player, that this Release, its significance and the assumption of risk set forth herein has been explained to and understood by the Player, that the Player is in good physical condition and that the Player has no disability, impairment, or aliment that would prevent the Player from engaging in active or passive exercise that will be detrimental or inimical to the Player’s health, safety, comfort or physical condition. It is recommended that every parent or guardian consult his or her own physician before permitting their child or ward from beginning any exercise program. The undersigned further acknowledge that professional medical help may not be readily available while the Player is participating in Club activities and hereby release TNVBC from any claim whatsoever on account of first aid treatment or services rendered to the Player during Player’s participation in Club activities. Members shall not be relieved of their obligations pursuant to the TNVBC Club Participation Agreement and no deduction or allowance from any payments required to be made thereunder shall be permitted by reason of the absence or withdrawal of the Player from the Club or by reason of the Member’s failure to attend or use the Club’s facilities.

Athletes involved in any activities offered by Tampa North Volleyball may be photographed or videotaped during training and competition. The undersigned hereby consents to the use of these photographs and/or videos without compensation, on the Tampa North website, print publications, online publications, presentations, and social media.

This Release shall be governed by the laws of Florida, supersedes all prior oral or written representations or communications between the parties and constitutes the entire understanding of the parties regarding the subject matter of this Release, and may only be modified or amended by a written supplement signed by all parties.

Notice to the Minor Child’s Parent or Guardian: Please read this form completely and very carefully. You are agreeing to allow you minor child to engage in potentially dangerous activities. You are agreeing that, even if the released parties use reasonable care in connection with the activities described herein, there is a possibility that you child may suffer serious injuries or death by participating in those activities because there are certain dangers inherent in the activities which cannot be avoided or eliminated. By signing this Release, you are, among other matters, giving up your child’s rights to recover from the released parties for any personal injury, including death to your child or any property damage that may result from the activities. You have the right to refuse to sign this Release, and the Club has the right to refuse to let your child participate if you do not sign this Release.
